How to Get PhilHealth ID
Philippine Health Insurance Corporation, known as PhilHealth, is government-owned health insurance and controlled corporation in the Philippines. Before claiming any PhilHealth benefits, members have to provide their Member Data Record or PhilHealth ID. Usually, members don’t get the PhilHealth ID even if it’s free of charge because most transactions only need to provide their membership number for most transactions.
*For Employees and Voluntary Members
Go to the nearest PhilHealth office with your two valid IDs or a photocopy of your NSO Birth Certificate. Fill out a request form and check the PhilHealth Identification Card box. Submit your paper and wait for your ID to be issued.
*For Senior Citizens
Suppose you applied via the Office of Senior Citizen Affairs (OSCA). In that case, you need to fill out two copies of the PMRF and submit both accomplished forms to the OSCA in the city or municipality where the elder lives.
If you applied at the PhilHealth office, fill out two copies of PMRF and attach a 1×1 photo of the senior citizen requesting the ID. Present the senior citizen’s OSCA-issued ID or any valid government ID. Submit the accomplished PMRF and wait for the ID to be released.

Regular PhilHealth ID vs. PhilHealth Insurance ID card
A regular PhilHealth ID card is a paper ID that you can use when claiming inpatient or outpatient hospital benefits, and you can claim this without charge at any PhilHealth offices, or you may request your HR office to process this for you.
It is one of the Philippines’ valid IDs but cannot be used for banking or government transactions. It is limited to getting essential services or filing insurance claims at hospitals.
The Philhealth Insurance ID Card is a digitized ID issued to employed and active members. It’s a PVC plastic card and can cost some fees and has more use than the regular ID card because it is a valid ID.
You can also enjoy exclusive member discounts and deals at selected drug stores, laboratories, drug testing sites, and optical shops.
It can claim it at PhilHealth offices and costs P90, but if you want to declare a dependent on your ID card, you need to go and pay P358 at any Bayad Center outlet. Two valid IDs of your dependent are required to process.
Try to check on the nearest PhilHealth offices if the digitized ID card is available because it is not available for distribution in all regions.
How to Replace Lost ID
You need to go through the application process again and submit the exact PhilHealth membership requirements, plus an affidavit of loss.
The same goes if you have a damaged ID then, surrender the damaged one and your new application.
